Located in Chatsworth, California next to West Hills Hospital and Medical Center lies Stoney Point Healthcare Center. Our healthcare center has been providing personalized care since 2015. We provide skilled nursing and therapeutic services uniquely tailored to meet the needs of residents diagnosed with Alzheimer’s disease and other forms of dementia.
Our team of skilled, dedicated, and caring clinicians looks forward to continuing the tradition of delivering quality care to everyone who enters through our door. We are genuinely interested in our residents health and will do our best to ensure they have everything they need for a quick recovery.
Often the transition to a skilled nursing facility is challenging. At Stoney Point Healthcare Center, we strive to ease the process by providing an environment where residents and guests feel informed and comfortable. Our staff works with each resident and family to encourage active participation in establishing rehab goals and plans of care.
Our facility’s secured perimeters afford our residents with the comforts of a spacious outdoor environment, conveniently encompassed by the Santa Susana Mountains. To preserve our residents’ dignity, our facility does not restrict residents to specific areas or units.
